These Terms of Use (“Terms”) govern your use of the Forms App (“App”), provided and operated by:

By downloading, accessing, or using Forms App, you agree to these Terms. If you do not agree, you must not use the App.
1. Acceptance
You must be at least 18 years old, or the legal age in your jurisdiction, to use the App. By using the App, you affirm that you meet this requirement and agree to comply with these Terms.
2. Subscription and Payments
- Forms App may offer subscription plans (weekly, monthly, annual).
- Subscriptions automatically renew unless canceled at least 24 hours before the end of the current period.
- Payment is charged through your Apple ID account at confirmation of purchase.
- Subscription management (including cancellations) is done through your App Store account settings.
3. Free Trial
- A free trial may be available for certain subscriptions.
- Trials automatically convert to paid subscriptions unless canceled at least 24 hours before the trial ends.
- If you purchase a subscription during the trial, unused trial days will be forfeited.
4. Intellectual Property
Forms App and all related content, software, and technologies are owned exclusively by SMM Service, s.r.o., and are protected by international copyright and intellectual property laws. You may not copy, distribute, reverse engineer, or modify any part of the App.
5. User Content
- By uploading content to the App, you grant SMM Service, s.r.o. a non-exclusive, worldwide, royalty-free license to use, reproduce, modify, and display such content.
- You are solely responsible for the legality, reliability, and appropriateness of any content you upload or share through the App.
6. Privacy
Your use of Forms App is also governed by our Privacy Policy, available separately within the App and the App Store.
7. Limitation of Liability
Forms App is provided “as is” without warranties of any kind. SMM Service, s.r.o. is not liable for any damages arising from your use or inability to use the App, including, without limitation, indirect, consequential, special, or incidental damages.
8. Termination
We reserve the right to suspend or terminate your access to Forms App at any time, without notice, for conduct violating these Terms or harmful to other users or us.
9. Modifications
We may modify these Terms at any time. Continued use of the App after modifications constitutes your acceptance of the revised Terms.
10. Governing Law and Jurisdiction
These Terms are governed by the laws of Czechia, without regard to conflict-of-law provisions. Any disputes will be subject to the exclusive jurisdiction of the courts located in Czechia.
